BMI:30~31 (BMI低於18.5者禁止發減肥文)
體脂率:29~29.8 (若不知道請至保健室、健身房、醫院、藥妝店等處測量,未填必砍)
(女性請註明體脂率量測之際為生理期前、正值生理期、或生理期後)

參考照片:(可免填,但抱怨自己太壯或肌肉太大塊者必填)
三餐內容:熱量估算易受餐飲的原物料、份量、製程手法影響,請儘可能量化並描述
例:使用直徑「30~40mm之貢丸或桌球體積」為度量衡參考標準
吃了2貢丸炒A菜,油度還好;吃了3貢丸魯肉.....etc...
早餐:一杯無糖豆漿 + 一個便當盒的青菜 or 一顆大饅頭+一點剩菜+無糖豆漿
午餐:公司水果餐(西瓜/芭樂 /柚子各兩三大塊+豆花+半塊麵包
)
晚餐:半塊麵包 or 一塊地瓜 or 御飯糰 +無糖豆漿
其他:一周 2杯咖啡,一周有兩餐午餐會正常吃(可免填)
日常作息時間:06:30 起床 11點半睡 (睡眠、工作、...等時間)
生活型態:上班族 (上班族、學生、體力工作者、運動員、...)
健康狀況:(是/否)否
醫師是否告訴過您,您的心臟有問題,您只能作醫師建議的運動?
當您活動時是否會有胸痛的感覺?
過去幾個月以來,您是否有在未活動的情況下,出現胸痛的情況?
您是否曾因暈眩而失去平衡或意識的情況?
您是否有骨骼或關節問題,且可能因活動而惡化?

運動習慣:(有氧運動請「務必」寫明心跳數、持續時間長短、及相關運動史)
例1:慢跑40分鐘,心跳數約130~140左右,持續1年
例2:騎腳踏車一小時,心跳數約130~150左右,持續1個月
之前在家平均2天一次 原地跑40min + 有氧體操40min +跳舞40min
目前去健身中心跳有氧舞蹈60min+踏步機30min or 有氧舞蹈30min +拳擊 60min

1.目前持續約一個月,電子體脂計數字顯示 變重1~2 KG 體脂少約1%,
但是自己看外觀感覺不出來有變瘦
2.水果吃多了會覺得冷,附近的自助餐又太油,不知道能夠選什麼?
3.我知道重訓對減肥的幫助很大,但個人對重訓較無興趣 只做仰臥起坐 /
俯地起身有效嗎
4.運動後約9~10點在吃晚餐會太晚嗎? 因我運動前吃 會不太想運動
