[前言]
先強調,這並非嚴謹的科學研究,
只是個案報告,內容僅供參考。
希望大家看了以後,
若自己或身邊的認識人,有躁鬱症的問題,
可以藉此多一份警戒心而已,
並沒有任何歧視或引戰的目的,
希望有心人不要大作文章。
[個案報告]
個案今年約27~28歲,
推測是在服完兵役後,躁鬱症開始發作,
導致工作能力下降、生活作息紊亂、社交認知變差,
而且較無法控制自己的情緒。
由於前述原因,難以找到穩定的工作,
所以最後選擇去開計程車,
因為工作時間彈性,不需要與人頻繁互動,
也不容易被發現有精神問題,進而導致被老闆開除。
後觀察個案病情相對穩定,
能持續計程車之工作,前後約3年,
雖收入不甚理想,但靠著家中支應,生活倒也無虞。
然個案約從1年前開始健身後,病情就逐漸惡化,
生活作息嚴重紊亂、日夜顛倒,在PTT與FB上異常活躍,
但想法固著、言談鬆散,且變得易怒與高自尊。
除了在網路上頻繁與人叫囂謾罵,
路上看到有人交通違規,會堅持尾隨在後,
想用行車紀錄器拍下證據後檢舉,
由於該行為的頻率實在太高,幾乎每天都有,
因而嚴重影響到自己計程車的營運。
另外注意到,個案的認知能力明顯下滑,
漸漸無法判斷,怎樣的行為才算交通違規,
也為此多次和網友爭吵,甚至口出惡言。
隨著個案病情惡化,對家人與社區的干擾越來越大,
也開始出現誇大妄想。
比如雖然自己體脂很高,舉重成績也一般般,
但依然堅信自己練健身練得很壯,
全身都是發達的肌肉。
或者想把計程車拿去抵押,宣稱自己很會做生意,
拿到現金後可以創業賺大錢。
同時個案也出現過分的購買慾,
如一次想買20公斤的乳清,每天當水喝。
由於精神狀況的異常,以及頻繁向警局投訴交通違規,
終於引起相關單位注意,並且廢止他的計程車執照,
而這對個案無異是相當巨大的打擊。
因為個案當時在發病後,對於自己的失能相當自卑,
覺得自己沒救了,做什麼事情都會失敗,
永遠也找不到工作了。
然在費盡九牛二虎之力,終於取得計程車營業許可後,
個案獲得了寶貴的成功經驗,
除了象徵社會對他的接納,也重新建立自我價值感。
如今被剝奪這項「榮耀」與「肯定」,
不但讓個案產生嚴重的挫敗感,
也更加惡化了他的躁鬱症,產生了更強的攻擊性,
在PTT和FB上,脫序的言論也更加明顯......
[結論]
眾所皆知,重訓本身是一種壓力,
除了會活化交感神經,也可能影響體內的激素分泌。
在身體可調節的情況下,這些壓力能產生「去敏感化」的作用,
讓交感神經與激素分泌變得更穩定,
人也更能適應外界的變化。
但對於身心靈皆不穩定的躁鬱症患者來說,
這樣的壓力有可能造成平衡崩潰,
反而使得病情惡化。
故若躁鬱症患者要重訓,
還是應該循序漸進,量力而為,
強度需要較一般正常人更低。
且隨時注意自身狀況,
若有病情變化,務必及早回診,
積極尋求醫療協助。
