一名廿七歲女子拚命減肥,不運動光靠節食瘦身,導致肌肉大幅流失,年紀輕輕就失能,
無法提重物,連一層樓梯都爬不動,被攙扶坐輪椅送急診,檢查發現她的下肢肌肉量還不
如七十歲的阿嬤。
萬芳醫院家醫科醫師陳柏臣表示,該女身高一百六十公分,僅卅多公斤,整個人很虛弱,
皮膚皺垂、老態盡現。女子說已減肥五年,但近年明顯感覺體力很差,平時出門下一層樓
梯,上班時爬一層樓梯,都感吃不消。
電腦斷層掃描發現,女子下半身大腿肌肉量微少,還不如七十歲阿嬤,蛋白質及電解質嚴
重不足,後來注射點滴,配合飲食及運動訓練肌耐力,雖然肌肉量增加,但仍無法回復到
年輕人該有水準。
陳柏臣說,愈來愈多年輕女性為減肥完全不碰肉,也不運動,造成蛋白質缺乏,肌肉快速
流失;加上部分女性減肥嘴饞常吃零食,導致胺基酸進入體內代謝為脂肪。雖然表面上體
重變輕,但肌肉與骨骼卻因營養不均衡嚴重流失,肌肉萎縮退化、體能差,走路或簡單動
作無法平衡,如同失能老人,「手抬不高,腳無力。」
「體重不是重點,身體密度才是關鍵。」陽明大學運動健康科學研究中心主任陳俊忠說,
許多年輕女性飲食不正常,身體像「泡芙」缺乏肌肉支撐,隨便動一下就累。
陳俊忠說,檢測身體密度除了計算身體質量指數(BMI,即體重除以身高公尺的平方),
還可單手握力及蹲坐測試觀察上、下肢肌肉,如果老人家單手握力無法超過廿公斤,代表
搭公車或拄柺杖都有問題;年輕女性單手握力至少應達廿五公斤以上;男性約四十至六十
公斤。
陳俊忠說,平時透過負重、爬山或爬樓梯可訓練下肢肌肉,提重物可練上肢肌肉,循序漸
進,約四至六周可見成效。
