棘下肌(infraspinatus)基本介紹
從內側三分之二棘下肩窩(infraspinus fossa)
連接至肱骨大結節(great tuberosity)的中端
同樣由肩胛上神經(suprascapula nerve)所支配
主要的活動為肩關節外轉(external rotation)
以及在活動過程中穩定肱骨頭(humerus head)於肩臼(glenoid fossa)內
在肩膀外展低角度相對於高角度時,棘下肌是更為有效率的外轉肌
因此肩膀外展零度時的外轉是最能獨立訓練這條肌肉的動作

小圓肌(teres minor)基本介紹
從上三分之二肩胛骨外側緣(lateral border of scapula)
連接至肱骨大結節(great tuberosity)的後端
由腋神經(axillary nerve)所支配
主要的活動也是肩關節外轉(external rotation)
以及在活動過程中穩定肱骨頭(humerus head)於肩臼(glenoid fossa)內
產生的外轉力矩遠小於棘下肌,在各肩關節角度下也沒有顯著的差異
棘下肌+小圓肌綜合鍛鍊方式(誘發肌肉活動由高至低排序)
雖然部分單獨訓練可以分別達到最大刺激
但依照其功能性考量,我認為同時刺激的訓練比較具有功能性上的實質意義
1.側躺肩外轉(side-lying with ER at 0 abduction)
唯一兩塊肌肉都會超出80%最大肌力的動作
在手肘的地方放一個毛巾卷讓手臂維持與地面水平可以讓動作更好

2.T字運動/反向飛鳥+拇指朝上(T's exercise/prone fly+ER)
這個運動應該很多人都有練
是一個很不錯的背肌綜合訓練
對三角肌(deltoid)跟斜方肌(trapezius)也有很好的刺激效果

3.T字運動/反向飛鳥+拇指朝下(T's exercise/prone fly+IR)
跟上面一樣的動作只是改成肱骨(humerus)內轉
刺激的程度較低,但對於三角肌有比較強的效果
4.站姿肩外轉(standing ER with 90 abduction)
在物理治療非常常用的一個姿勢
同時也會對棘上肌與肩胛下肌(subscapularis)達到50%以上的刺激
